 Housing Transition Navigation Services assist members in identifying, securing, and preparing for housing. Services are individualized and may include:

• Housing assessments and development of a Housing Support Plan
   
• Assistance with housing searches, applications, and documentation
   
• Coordination with landlords, property managers, and housing authorities
   
• Support with reasonable accommodations and fair housing protections
   
• Connection to community resources and public benefits

Housing Deposits provide one-time financial assistance necessary to secure housing, when all other funding sources have been exhausted.

Allowable supports may include:

• Security deposits
   
• First month’s rent (when authorized)
   
• Utility deposits or activation fees
   
• Essential household items required for move-in
   

Housing Deposits are authorized only when the member is actively engaged in Housing Transition Navigation or Tenancy and Sustaining Services and when documented as medically appropriate. 

Housing Tenancy and Sustaining Services support members in maintaining safe and stable housing after placement. Services may include:

• Education on tenant rights and responsibilities
   
• Coordination with landlords to prevent eviction
   
• Support addressing behaviors or issues that may jeopardize housing
   
• Development of budgeting and financial literacy skills
   
• Connection to ongoing health, behavioral health, and social services
   

HTSS is designed to promote long-term housing stability and prevent returns to homelessness.
